Instructions for sending a Workers Compensation Prescription to The Compounding Shop

Please fill out the secure form with the patient’s/client’s information, including Claim number, date of injury, delivery method, your insurance company name and contact information. You can attach a copy of the prescription if necessary.

The Compounding Shop will acknowledge receipt of the prescription, and send pricing and availability information back to you within one business day (Monday through Friday, excluding holidays) via secure email notification, so please check your email for this. If necessary, please add www.gotocompoundingshop.com to your white list.

Sign the pricing agreement and return to The Compounding Shop by using the button on the form.

The Compounding Shop will contact the patient for pick up or delivery instructions.
